What a referring physician receives back when a patient joins.

I grew up in a family of physicians, and I recall how frustrating and disjointed the referral process sounded from their side. They would send a patient out with good intentions, and the next thing they heard was whatever the patient remembered to mention at their next visit. There was no solid follow up plan or time to sort one out when a doc was outside their network - formal or informal.

That’s not good enough. So we made it our job to support a better outcome for you and your patient. Here's what lands on a referring physician’s desk when a patient joins our team:

– Confirmation they’ve started, so you’re not left wondering whether they followed through.

– Their starting point in one place: body composition, mobility, strength, and the labs you already ordered.

– The plan, written in language you’ll recognize: what we’re working on, what we’re watching, and what stays in your lane.

– A SOAP note each month.

– A heads-up when something we see belongs with you. We don’t diagnose or touch medications. If it’s medical, it comes back to you.

None of this is complicated. It’s just rarely done.

A referral shouldn’t feel like letting go of a patient. It should feel like adding people to their care team.

Physicians, what’s the one thing you wish you heard back after a referral?
